This form is completed by families for reduced price lunch under the Federal School Lunch Program. A new form must be submitted annually to qualify for this program.

Breakfast
LHS and LJHS students - $1.50
Elementary school students - $1.25
Milk at all levels is $0.45.
Reduced price for breakfast is $0.30
Reduced price for lunch is $0.40
